LESSON AGREEMENT FOR LORNA MUSIC
Dear Students and Parents/Guardians,
We at Lorna Music are very happy to have you as a student. We hope that each of you will learn and reach your potential as a musician.
The following Agreement is our policy at Lorna Music. Please read carefully and ask any questions you may have before you sign the agreement.
1. Checks should be made payable to your instructor.
2. There is a set fee of $72.00 per month for four, half-hour lessons per month. During months with five lesson days, no lesson will be given on this fifth week. Payment is due at the time of the first lesson each month. No money refunded.
3. We charge for missed lessons. If a student desires a "makeup lesson" arrangements are to be made with your instructor.
4. To discontinue lessons, Lorna Music should be notified at least one week prior to student's termination.
5. It is understood that you are still enrolled until you tell us you are dropping.
6. If payment is due and we are not notified of your termination, you will be charged for missed lessons.
7. By signing below, you are representing that you are at least 18 years old or that you are the parent and/or legal guardian of the named student and agree to the terms and conditions contained herein.

SOME OTHER HELPFUL THINGS TO KEEP IN MIND
Scheduled lessons should be paid for when they are scheduled. Lessons are to paid by the month - on or before the first lesson of that month. Monthly lessons are $18.00 each. If the month’s lessons are not paid on or before the first lesson of the month, a late charge of $5.00 per each week may be added to that month’s bill. If you will not be here for the first lesson of the month, please call your instructor or Lorna Music so you are not inadvertently removed from the lesson schedule. Students will not be charged for holidays when the store is closed. Holidays include New Years Day, Memorial Day (Observed), July 4th, Labor Day, Thanksgiving, and Christmas Day. The store will close at 3:30 PM New Year's Eve and Christmas Eve. When a teacher is absent, a make-up lesson should be arranged with your instructor.
If a student needs to cancel a lesson, makeup lessons will be given if possible, but are arranged at the teacher's convenience. All lessons should be made up within 60 days of the lesson that was canceled or the makeup lesson will be forfeited by the student. There will be no refunds for lessons, only makeup lessons if proper notice is given. If a student misses the first lesson in a given month, he or she may still be charged for the entire month. If a student doesn't wish to schedule for an entire month they may schedule individual lessons at $23.00 each for some lessons that month. Lessons not scheduled of course have no makeup, and cancellations work the same as when the whole month is scheduled.
You may call Lorna Music during regular open hours to cancel lessons. Messages may be left on the answering machine when we are closed. The answering machine will date and time stamp the message. If there is no answer when you call, please call back a little later, there are 2 phone lines and the answering machine is probably answering another line. If two lessons are missed in a row with no cancellation notice, even if it is too late for a makeup, the student may be removed from the teacher's lesson schedule. Lessons may be paid for by cash, check, Master Card, Visa, or Discover.
